By sending this application, I consent to the processing of my personal data by FKA Furtek Komosa Aleksandrowicz Sp. k for marketing purposes, including receiving the FKA Newsletter via electronic means of communication.
1) The controller of your personal data is FKA Furtek Komosa Aleksandrowicz sp.k. with its registered office in Warsaw, at Plac Małachowskiego 2, 00-066 Warsaw (“FKA”).
If you have any questions concerning the manner, scope of processing of your personal data by FKA and your rights, as well as in order to exercise those rights, please contact us by e-mail at the following e-mail address: rodo@fka.pl or in writing to the above-mentioned address of FKA.
2) Your personal data are processed for the purpose of sending the FKA Newsletter, which includes i.a. information on important changes in law and events organized by FKA. The data are processed on the basis of your consent to be added to the database of the subscribers of the FKA Newsletter (Article 6.1 (a) GDPR). Providing FKA with personal data is voluntary, however if you choose not to provide your personal data, you request deletion of personal data or withdraw your consent this will prevent FKA from sending you information.
3) Your personal data will be processed by FKA for the duration of its economic activity, unless you withdraw your consent or request a removal of personal data, which will result in the deletion of data from the FKA Newsletter’s subscriber database.
4) The data is processed as part of the MailChimp system and is therefore stored on servers located within the territory of the United States of America (USA). The MailChimp system provider joined the Privacy Shield program and guarantees an appropriate level of personal data protection.
5) You have the right to:
a. access your personal data, including obtaining copies thereof;
b. data rectification, deletion, restriction of their processing, as well as the right to data portability – on the principles set out in Articles 16-20 GDPR;
c. withdraw consent at any time, which can be done by, for example, sending an e-mail to: rodo@fka.pl or clicking the link at the bottom of each newsletter; the withdrawal of consent does not affect the lawfulness of data processing which had been carried out on the basis of consent before its withdrawal;
d. lodge a complaint with a supervisory authority (which from the date of entry into force of the Act on the Protection of Personal Data of 10 May 2018 will be the President of the Office for Personal Data Protection), if personal data processing by FKA is considered to violate the provisions on personal data protection.
